SS-31 (Elamipretide, MTP-131, Bendavia) is a synthetic mitochondria-targeted tetrapeptide comprising four amino acids (D-Arg-Dmt-Lys-Phe-NH₂) that selectively concentrates at the inner mitochondrial membrane through high-affinity binding to cardiolipin. Discovered serendipitously by Hazel H. Szeto and Peter W. Schiller during opioid receptor studies in the early 2000s, SS-31 represents the founding member of the Szeto-Schiller (SS) peptide family and a first-in-class approach to mitochondrial medicine. The peptide was originally designed as a mu-opioid receptor agonist but was found to possess potent mitochondrial-targeting properties with negligible opioid activity. SS-31 has been the subject of over 200 peer-reviewed publications spanning cardiac, renal, neurological, and musculoskeletal research.

In September 2025, elamipretide received FDA accelerated approval under the brand name Forzinity for the treatment of Barth syndrome, making it the first FDA-approved mitochondria-targeted therapeutic. This approval was based on the TAZPOWER Phase 2/3 trial demonstrating +96.1 meter improvement in 6-minute walk test at 168 weeks (P=0.003). The peptide features an alternating aromatic-cationic motif creating an amphipathic structure essential for selective mitochondrial membrane targeting, with D-arginine at position 1 conferring aminopeptidase resistance and 2',6'-dimethyltyrosine (Dmt) at position 2 providing intrinsic antioxidant activity.
